We encourage you to contact Torri Attebury, of DSAF's Medical Outreach. In addition to its work with the medical community, the Medical Outreach team offers Parents First Call - a group of fellow parents, who share in the experience of an unexpected diagnosis and are trained to provide accurate information, support and perhaps most useful, personal insight.

HIPPA - Release of Information Authorization Form

HIPAA (Health Insurance Portability and Accountability Act of 1996) is United States legislation that provides data privacy and security provisions for safeguarding medical information.  A completed release form, signed by you, the patient/parent, allows a representative of DSAF to reach out via a Nurse/Doctor/Caregiver's referral with no violation to HIPPA.
